Overview

Vista Community Clinic is a private, non-profit, multi-specialty outpatient clinic providing care in a comprehensive, high quality setting. Responsibilities
  • Provide intensive case management and home visitation to program participants and assist in program activities and administrative support.
  • Implement identified evidence-based parenting education models with fidelity
  • Plan and implement family relationship building activities
  • Perform outreach and education activities within the community
  • Facilitate participant planning services to include interviewing, assessing, goal setting and referrals
  • Identify individual/family strengths that can be applied to overcome specific challenges
  • Demonstrate knowledge of program objectives and scope of work
  • Complete enrollment documents for each client referred by VCC and community partners into the program
  • Maintain ongoing contact with clients to build trusting relationships, provide support, serve as a referral source and monitor progress toward identified goals
  • Maintain accurate, current participant files including attempted and actual contacts, flex funds, and document required program data and instrument as required by funders
  • Develop and maintain a network of resources and services in the community to serve and assist with client needs
  • Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
  • Perform other duties as directed

Qualifications

Minimum

  • Bachelors degree in related field (psychology, social work, etc.)
  • Bilingual English/Spanish
  • Minimum of one year education, case management or outreach experience
  • Valid California drivers license, current vehicle insurance and reliable transportation
  • Availability and willingness to work evening and weekend hours as required

Required Skills/Knowledge/Abilities

  • Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities
  • Willingness to perform tasks outside the normal range of duties
  • Ability to present information effectively verbally and in writing to individuals and groups from various cultural, ethnic and educational backgrounds
  • Strong organizational, time management and interpersonal skills with ability to create a comfortable, supportive learning environment

Pay Range

$21.00 - $23.00
