Case Manager- Parent and Child Health (PCH)

Provide case management, home or field visits, coordinate services for program participants, and assist in program activities and administrative support.
Enroll participants based on program eligibility criteria
Document case management using the program templates (Service Plan, Case Notes, Referral Log, Contact Log, Consent Forms, and other forms as directed)
Monitor the coordination of services and documents additional needs identified for each participant
Monitor utilization of services and care
Encourage and monitor that participants complete the educational sessions
Maintain information on available resources and services in the county and region
Maintain electronic client files as defined by the program
Protect client confidentiality following program protocol by utilizing encrypted email and/or fax, and obtain consent to release information for communication with other agency partners
Administer surveys, screenings, or assessments as defined by the program
Minimum
- High school graduate or equivalent
- Five years case management experience or equivalent combination of education and experience
- Bilingual English/Spanish
- Valid California drivers license and vehicle insurance
Preferred
- Bachelor's degree in related field with two years case management experience
Required Skills/Knowledge/Abilities
- Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
- Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
- Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel
Pay Range
$20.00 - $21.00
